In an effort to better serve our residents with Disabilities, we would like to know any information that may help us when serving them. The information submitted on the form below will be uploaded to our “in-house” system and will appear to officers whenever we receive calls to the residence indicated. This information is only used by the Police and only used to ensure a proper response to assist those with disabilities. We ask that the information be updated whenever important changes need to be noted or a chance of address occurs.
